QUALIFYING CONDITIONS

To qualify for a patient registry ID card in Kentucky—whether as an in-state qualified patient, a minor qualified patient, or a visiting qualified patient—an individual must be diagnosed with one of the following qualifying medical conditions:

Debilitating Medical Conditions (For Adults)

  • ​Any type or form of cancer

  • Chronic or severe pain

  • Epilepsy or other intractable seizure disorder

  • Multiple sclerosis, muscle spasms, or spasticity;

  • Chronic nausea or cyclical vomiting syndrome

  • Post-traumatic stress disorder
