Registration Guide

This guide outlines the steps for registering under the Kentucky Medical Cannabis Program for four categories of registry ID cards. Follow these instructions to ensure a smooth application process.

 

 


In- State Qualified Patient

Eligibility Criteria

  • Must be a resident of Kentucky.

  • At least 18 years old.

  • Diagnosed with a qualifying medical condition such as cancer, chronic pain, epilepsy, multiple sclerosis, PTSD, etc.

  • Must not have been convicted of a disqualifying felony offense.

     

Required Documentation

  • Written certification from a registered medical cannabis practitioner.

  • A government-issued ID (e.g., driver’s license).

  • Attestation of information sharing.

Application Process

  • Obtain a Written Certification

    • Visit one of EZmedcard’s Kentucky offices, a registered medical cannabis practitioner for an in-person examination.
    • The practitioner will confirm your diagnosis and submit the written certification within 24 hours.
  • Complete the Online or Paper Application

    • Applications open January 1, 2025 on kymedcan.ky.gov.
    • Submit your application within 60 days of receiving the written certification.
    • Pay the $25 non-refundable fee via credit card or ACH/eCheck.
  • Notarize and Submit Signature Form

    • Download and print the signature form.
    • Sign the form in the presence of an authorized Notary Public and upload the notarized document during the application process.
  • Wait for the Decision

    • The Cabinet will notify you of approval or denial within 30 calendar days. Approved applicants will receive their registry ID card electronically within 5 calendar days.

Notes

  • The registry ID card will expire 1 year after issuance unless specified otherwise by your practitioner. A renewal process is required to maintain eligibility.

 

 

 

 

 

In- State Minor Qualified Patient

Eligibility Criteria

  • Must be a resident of Kentucky and under 18 years old.

  • Diagnosed with a qualifying medical condition confirmed by a practitioner other than the medical cannabis practitioner.

  • Custodial parent or legal guardian must provide consent.

  • Must not have been convicted of a disqualifying felony offense.

Required Documentation

  • Written certification from a registered medical cannabis practitioner.
  • Medical records from another practitioner confirming the qualifying condition.
  • Custodial parent or legal guardian’s statement of consent.
  • Notarized signature form signed by custodial parent or guardian.
  • Attestation of information sharing.

Application Process

  • Obtain a Written Certification

    • An authorized medical cannabis practitioner must provide a written certification after an in-person examination.
  • Gather Additional Documentation

    • Collect supporting medical records from another healthcare provider confirming the diagnosis.
    • A parent or legal guardian must prepare and sign a consent statement.
  • Complete Application

    • Applications open January 1, 2025 on kymedcan.ky.gov.
    • Submit all required information and documentation within 60 days of receiving the written certification.
    • Pay the $25 application fee.
    • Submit a notarized signature form.
  • Receive Card

    • After approval, the card will be issued electronically. Note that minor patients are prohibited from entering dispensaries and must have a designated caregiver for purchases.

Notes

  • Custodial parents or legal guardians must also register as the minor’s designated caregiver.

 

 

 

Designated Caregiver

Eligibility Criteria

  • Must be a resident of Kentucky and at least 21 years old.
  • Cannot have been convicted of a disqualifying felony offense.
  • May assist up to three registered patients, including one minor qualified patient (if applicable).
  • If assisting a minor qualified patient, must be the custodial parent, legal guardian, or conservator.

Required Documentation

  • Attestation form confirming caregiver-patient relationship.
  • Information about the qualified patient(s) they are assisting.

Application Process

  • Verify the Patient’s Status

    • Ensure the patient(s) you are assisting are eligible and have submitted applications.
  • Complete the Application

    • Register as a caregiver starting January 1, 2025 via kymedcan.ky.gov.
    • Provide the required documentation, including details about the registered patient(s).
    • Pay the $25 application fee.
  • Notarize and Submit Signature Form

    • Download the designated caregiver-specific signature form. Sign it in front of a Notary Public and upload to the portal.
  • Await Approval

    • Approval takes up to 30 calendar days. Once approved, you will receive your registry ID card electronically.

Notes

  • Caregivers must use their ID card to purchase and administer medical cannabis for the patient. Minor patients cannot enter dispensaries or possess medical cannabis.
