At ezmedcard, we have the ability to evaluate you for PTSD which is a qualifying condition in the state of Connecticut. Here are the steps to be evaluated for PTSD:

Step 1) This is a fillable PDF and you must complete all 20 questions by clicking on the pdf document.

Step 2) You then must download the PDF form with your changes and send it back to info@ezmedcard.com so we can review your PTSD Form to determine if your assessment may suggest a PTSD evaluation.

Step 3) EZmedcard will reach back out to you once they review your PTSD .pdf that you attached to your email.
